Why Generative AI Isn’t the Same as Agentic AI

If you’ve been following the rapid evolution of artificial intelligence, you’ve likely noticed a new term making headlines: agentic AI.

For many healthcare organizations, the distinction isn’t always clear. After all, if generative AI in healthcare can draft clinical notes, summarize patient information, or answer questions, isn’t agentic AI simply a more advanced version of the same technology?

Not exactly.

While generative AI and agentic AI share a common foundation, they serve different purposes. Understanding that distinction is becoming increasingly important as healthcare organizations evaluate how AI can improve operations, reduce administrative burden, and support clinical workflows.

Generative AI Helps Create Information

The introduction of generative AI transformed how many organizations think about AI.

Unlike earlier technologies that focused on analyzing data, generative AI creates new content in response to a user’s request. It can draft text, summarize information, generate images, write code, or answer questions based on the model’s training and the data it is permitted to access.

In healthcare, generative AI use cases continue to expand, including:

  • Drafting clinical documentation
  • Creating patient education materials
  • Summarizing visit notes
  • Assisting with medical coding
  • Answering administrative or clinical questions

These capabilities help clinicians and staff complete everyday tasks more efficiently, reducing time spent creating content from scratch.

However, generative AI remains largely reactive. It waits for a prompt, generates a response, and then waits for the next instruction. A person or downstream process decides what happens next. Its core strength is creating information, not orchestrating work across systems.

Where Agentic AI Differs

Agentic AI in healthcare often builds on many of the same AI technologies but introduces planning, context, and workflow orchestration.

Instead of responding to one prompt at a time, agentic AI is designed to work toward a goal.

It can break a task into multiple steps, gather information across systems, determine what needs to happen next, and complete portions of a workflow within predefined guardrails.

McKinsey describes healthcare AI agents as “virtual workers” that can work toward a specific goal using context, guardrails, and existing tools, while involving people when needed for oversight.

That shift—from creating content to coordinating work—is what makes agentic AI different.

From Tasks to Clinical Workflows

The difference becomes easier to understand when viewed through a healthcare lens. Imagine a clinician needs to obtain a prior authorization.

With generative AI, the system might draft the medical necessity statement or summarize the patient’s clinical history.

With agentic AI, the system could:

  • review payer requirements
  • identify missing documentation
  • prepare supporting information
  • initiate the authorization workflow
  • route the request for approval or submission based on policy
  • monitor status updates
  • notify staff when additional information, judgment, or escalation is required

The clinician and staff remain in control, but much of the administrative coordination happens automatically within established rules.

The same concept applies across many clinical workflows, including patient scheduling, care gap outreach, referral coordination, and post-discharge follow-up.

Rather than supporting one isolated task, agentic AI helps connect multiple tasks into a coordinated workflow.

Human Oversight Remains Essential

As AI becomes more capable, one principle remains constant: healthcare requires human oversight.

Whether organizations are using ambient AI, generative AI, or agentic AI, clinicians and staff continue to play an essential role in reviewing information, approving decisions, and maintaining accountability.

For agentic AI, this often includes:

  • governed workflows
  • approval checkpoints
  • audit trails
  • monitoring
  • exception escalation
  • operational boundaries

These safeguards help ensure AI supports healthcare teams while maintaining patient safety, regulatory compliance, and organizational trust.

It also aligns with emerging healthcare AI guidance from The Joint Commission and the Coalition for Health AI, which emphasizes policies, local validation, monitoring, and responsible use as organizations implement AI at scale.

The goal isn’t autonomous healthcare. It’s intelligently designed automation in healthcare that reduces administrative burden while preserving clinical judgment.

An Evolution, Not a Competition

It’s helpful to think about AI as an evolution rather than competing technologies.

Ambient AI in healthcare captures clinical context.

Generative AI transforms that information into useful content.

Agentic AI uses information and context to help coordinate work across people, systems, and processes.

Each builds upon the capabilities that came before it, allowing healthcare organizations to adopt AI in ways that match their operational needs and comfort level.

The Right AI for the Right Job

As AI continues to evolve, the conversation should move beyond which technology is “better.”

The more important question is how each type of AI can solve the right problem.

Generative AI helps clinicians and staff create information faster. Agentic AI helps organizations coordinate increasingly complex workflows more efficiently. Together, they represent a more integrated stage in healthcare’s AI journey, focused on reducing administrative burden, improving coordination, and enabling care teams to spend more time where they’re needed most: with patients.
